How to Submit a Service Request

To submit an online request, follow the steps below.

  1. Select the Services link at the top of the page.

  2. When you arrive to the Service Catalog page, you will see some service request categories. Select the appropriate category for your request. 
     
  3. Once you select your category you will see a breakdown of choices pertaining to that selection. Select the appropriate category (example: Communications; Email) and continue until you see the service you wish to request.
     
  4. This page provides details about the service request. If this is indeed the correct request, click on the Request Service button. Otherwise please follow the instructions/links provided on how to get the service.

  5. The request form appears and now you can supply the details for your request. Some of the fields are required (*) while others are not but please be sure to include as much information as possible as this will aid in completing your request as quickly and efficiently as possible. 
     
  6. Once you have filled out the form. Click on the Request button to submit your request.

  7. A confirmation page will appear with your ticket number.  You can then close the tab or continue to other options.

  8. An email will be sent to you referencing your opened request.  Please save this email as it will provide you with your ticket information.
